The incumbent is responsible for comprehensive care including ordering and interpreting labs, ordering and refilling prescription medications, monitoring patients for adverse outcomes, providing disease state education, compliance counseling and other activities required to optimize clinical outcomes. The clinician will maintain competency in utilization of point-of-care testing devices and monitoring equipment as indicated for disease state management. The Pharmacist provides oversight of student and resident pharmacists as assigned. -AGE SPECIFIC CARE: Provides age and culturally appropriate assessment, interpretation of clinical and laboratory data and develops and implements age appropriate interventions and plans of care including education within the parameters of his/her position responsibilities.EDUCATION/TRAINING:Bachelor's degree in Pharmacy or a PharmD Degree required.
